Position Overview

As a Home Health Physical Therapist, you'll deliver personalized therapeutic care to adult patients while enjoying professional autonomy and work-life balance. This role combines clinical expertise with relationship-building skills in a dynamic home care setting.

Core Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans
